Perry Meridian High School Athletics

All Falcon Student-Athletes need an up-to-date physical to participate in athletics for the 2025-2026 school year. The Falcon Physical Night is July 23rd, 6 pm to 8 pm for $20 at Perry Meridian High School. This event coincides with the club fair and incoming freshman parent meeting to provide greater access for our Falcons. Any Perry Township student-athlete in grades 6 through 12 can receive a sports physical during this opportunity. Must have pages 2 and 3 (Medical History) completed to see the doctor during the physical night. You can print this form from Final Forms instead.
